LEAGUES & TOURNAMENTS
All game requests must be submitted one week prior to
the first day of the league. All schedule requests made
after this date will be charged a fee of $10. If a schedule
request is made within 2 business days of already
scheduled games, a fee of $20 will be charged.
WAIVER Form/Individual Registration
Individual/Waiver form must be filled out by Individuals participating at Players Choice of the Fox Cities.
Team Registration Form
Form must be filled out for Teams participating at
Players Choice of the Fox Cities, LLC
